Medicare Part B (Medical Insurance) Annual “Wellness” Visits
If you’ve had Medicare Part B for longer than 12 months, you are entitled to an “Annual Wellness” screening visit. Your FMWC provider will help to develop or update a personalized prevention plan to prevent disease and disability based on your current health and risk factors. We will ask you to fill out a questionnaire, called a “Health Risk Assessment,” as part of this visit. Answering these questions can help us develop a personalized prevention plan to help you stay healthy and get the most out of your visit. This form can be filled out at home and brought with you to the visit, if you wish. Along with an update of your general information, this visit includes:
- A review of your medical, surgical, social and family history
A review of the current list of other providers involved in your healthcare
Measurement of your height, weight and blood pressure
An assessment of cognitive impairments such as possible hearing impairments and fall risks
A discussion of home safety
A list of risk factors and treatment options for you
Personalized health advice
A screening schedule (like a checklist) for appropriate preventive services. Get details about coverage for screenings, shots, and other preventive services.
